What Will The ID Cards Look Like?
FACT! Did you know ID cards are often accidently thrown away as junk mail? It may not be
obvious that the envelope includes ID cards. Open all incoming mail from Kaiser.

Northern California Laminate ID Card Transition (Starting Oct 2020)
Transition away from plastic ID card with new laminate.
Digital ID card will also be enabled when kp.org account is set up
Any request for replacement card will receive new laminate version
Plastic, laminate or digital can be used all Kaiser locations

If members are newly enrolling in coverage effective January 1st ID cards will be received in late
December.
If you need to access care before receiving your ID card, please follow the instructions below.
Kaiser Medical: If you need to visit the doctor or have a medical treatment scheduled please
contact Kaiser Member Services at (800) 464‐4000 for assistance. They can provide you with
your new ID card number.
Access health plan information anytime/anywhere via Kaiser’s website and mobile app so that
you can:
Print/view temporary ID card/Digital ID
View claims
Register for the Mobile application (App and Play store)
Access additional resources

You can visit your doctor without your ID card by providing the policyholder’s name, SSN and plan
enrolled in (e.g., Kaiser). The office can call Kaiser’s Customer Service to verify eligibility and
benefits.

Kaiser Senior Advantage Plan (KPSA): If you need to visit the doctor or have a medical treatment
scheduled please contact Kaiser Member Services at (800) 443‐0815 for assistance. They can
provide you with your new ID card number.
